The Support Through Housing Team provides hard to house individuals and families in our community with the structure and support they need to address critical issues necessary to obtain and maintain permanent housing. Services are also provided to individuals at risk of losing their housing due to their circumstances. A full time outreach coordinator supports and maintains a caseload of 30 to 40 clients and collaborates with the 13 partner agencies and other support agencies as deemed necessary. The collaboration of the outreach coordinator and the partner agencies allows for wrap around services connected to the individuals and families served.
